Ho Sobia, Alli is right, it is better for you to find the specific information that applies in your region, but this is the form available for people who apply in El Salvador, for instance, and for other countries and regions I am pretty sure it is almost the same information that you will need to fill out, and maybe slightly different but almost the same, so this is a great way for you to have a clear idea about the application form:

Some of you have been commenting about volunteering. That is one of the best parts of the program and I strongly recommend you to participate in as many volunteering activities as you can. In the UGRAD program volunteering and a short internship are requirements. Everyone will work sooner or later in life. But as for volunteering, that is a culture you need to develop and the UGRAD program encourages to do that. You will enjoy it and it will give you happiness and satisfaction to help others.

Hi Talal, It is true that we love volunteering. I have volunteered at many places after my Ugrad program.  From there, I met and made new friends and I volunteer at least once a month at 1-day program such as raising funds organization.  I am sure you will find plenty of organizations need our little help which means a lot to everyone.  Let's keep it up! :)
